Self directed support

At the heart of self directed support is the idea that you are the best person to say what is right for you. We want to help you take as much control over your care and support as you feel comfortable with.

We want to support all our residents to stay healthy and actively involved in community life for as long as possible. When this is not possible, we want to help you to retain the best quality of life you can.

If you have ongoing personal care and support, we can help by working out, with you, your individual needs and calculating the cost to meet those needs. If you are eligible for financial support, we'll contribute towards the cost of your care. You can then choose how to meet your needs in an appropriate way that is agreed by us.

Even if you pay for all of your own care, we can give you information, advice and support to help you to stay in your own home or move into a care home.

The idea behind contributing towards the cost of your care, instead of providing you with services directly, is that we want you to be in charge of how your support is arranged and have maximum control over how the money is spent. You can also choose to let us arrange your care and support.

How it works

After you’ve contacted us, we’ll complete a self directed assessment questionnaire with you to identify which of your care and support needs qualify for our support.

We'll then calculate how much we think it will cost to meet your needs. Once you've completed a financial assessment we'll be able to tell you how much we can contribute towards this.

If you qualify for financial support from us, we'll work with you to plan your care and support and what you will need to do if you wish to take control of the money we are able to give you towards your care.

If you are using your own money to fully pay for your care and support, we will still offer you help to identify your needs and help you plan and arrange your support.
