Assessment

The first thing you need to do is contact us either by email catreferral@buckscc.gov.uk or by phone 01296 383204. You can also ask your GP or health professional to refer you. We'll then arrange for you to have an assessment to identify which of your care and support needs qualify for our support.

After your support and financial assessments we’ll be able to tell you how much we can contribute to your Personal Budget. You can then choose how to meet your needs in an appropriate way that is agreed by us.

The assessment uses a questionnaire about your life. It covers what kind of support you need and most importantly, what you want to achieve. It’s designed to be simple to complete, each question has a choice of answers so you can pick the one that best fits you. One of our care management staff will help you to fill this out. You can ask people who know you well to be at the assessment eg family, friends, an advocate or anyone else you’d like to help. This sample questionnaire will give you an idea of the sort of questions you'll be asked in your assessment.
