Assessments
If you have a disability or serious illness which makes it difficult for you to live safely and independently, we can assess you to see whether we can offer you some help.
Initial assessment
We ask you about your situation and fill in an initial assessment with you. This can be done by phone or at one of our local offices. We will then be able to say whether you need a full assessment and will arrange a convenient time and place with you.
Full assessment
This is when you, the people who help you and a care manager work out the needs you have. It includes:
- what you are able to do on your own
- What you need help with
- what help you have already
- what support you need now, to live independently, to stay at home, or be where you want to be.
A family member, a friend, or an advocate is welcome to come with you. A care manager will then let you know if we can help you.
If we assess any of your needs as 'critical' or 'substantial', you will be eligible for help from us. Currently, we cannot offer services to people whose needs we assess as 'moderate' or 'low'. If it is not possible to offer you services, we will put you in touch with local organisations that may be able to help.
Carers
If a friend or family member provides care for you, they can be assessed as well. A carer's assessment looks at how we can help them care for you. We can assess you both together or separately, depending on what you or the carer prefer.
